Pramlintide is a synthetic analog of human amylin, a pancreatic beta-cell hormone co-secreted with insulin. Approved by the FDA as an adjunct to insulin in type 1 and type 2 diabetes, it is the only non-GLP-1, non-insulin injectable therapy for diabetes. It slows gastric emptying, suppresses glucagon, and enhances satiety.

Mechanism of Action

Mimics amylin, binding to amylin receptors in the brainstem (area postrema) and periphery. Slows gastric emptying, suppresses postprandial glucagon secretion, and enhances centrally-mediated satiety. These effects reduce postprandial glucose excursions and promote modest weight loss.

Type 1 diabetes: initiate at 15 mcg SC before major meals, titrate to 30-60 mcg. Type 2 diabetes: initiate at 60 mcg SC before major meals, titrate to 120 mcg. Mealtime insulin doses must be reduced by ~50% when initiating to prevent hypoglycemia.
